Community Impact Grant Program – Full ProposaL

Thank you for your interest in the United Way of the Mid-Willamette Valley’s 2019 Community Impact Grant Cycle. We are pleased to invite your organization to submit a full proposal. Please use the form below to submit your proposal. The deadline to submit the full proposal is September 15, 2019 at 5 p.m. PST.


    - Brief description of your project/program and its projected community impact - Requested funding amount and what funds will specifically be used for
    - Describe your project/program’s timeline (approximate start and end date) - Describe the impact your project/program will have on the community - Describe your project/program’s plan for measuring impact - Please share milestones and/or measurable outcomes
    - Organizational Description (mission and key programs) - Brief needs statement detailing the community issue to be addressed - Description of the community(ies) to be served - Explanation of the project/program activities - Please describe how your organization addresses Diversity, Equity and Inclusion (DEI)
