Info Session: Service Integration Teams in Marion County (Salem)

Salem Health and other partners in Marion County are interested in developing a community-based Service Integration Team (SIT) model for Marion County. The Steering Committee is hosting three information sessions to explain how collaborative community agencies can apply to become a Service Integration Team.

Service Integration Teams share information about resources, seek help with unmet needs, and support community-wide service oriented events. This approach is based on the highly successful Service Integration Model started in Polk County and recently adopted by Yamhill County partners.

Salem Health will provide start-up funds and administrative support for two pilot sites: one in a rural area of Marion County and the other in Salem-Keizer. SIT meetings will be held monthly in the two selected communities and will serve individuals and families living within the Marion County Public High School catchment area associated with your group. The SIT partners will align resources, relationships, and when necessary use team funds to meet the needs of local families and communities.

The Marion County SIT Steering Committee is looking for communities interested in developing a Service Integration Team. The committee will select one area that serves a high school catchment area within the Salem-Keizer School District and one rural high school catchment area in Marion County.
The selected communities will receive team support, facilitation and funding.

Please join us to learn more about Service Integration Teams and how your community can apply. The Request for Application will be presented at the information sessions.  Attendance is required to apply to become a SIT.
